How to apply

Applying for one of our services

Our applications typically come from Clic Sargent social workers and medical practitioners, but on occasion from the families or young person themselves. The hospital the applicant attends must be registered with Josie’s Dragonfly Trust. Registered organisations are given a password to download the application forms below.

A list of registered hospitals can be found here.

If you are not on the list and would like to register your organisation to receive support from Josie’s Dragonfly Trust, please call 01434 602043 or email

If you know a young person who is currently under hospital/hospice care who you think would benefit from the support we offer, please contact the charity to discuss the application and registration process.

Please read the application guidelines below very carefully as we do not want to disappoint any families/individuals who do not fit the criteria.

Register your organisation for an Arts and Crafts Box

Josie’s Dragonfly Trust currently distributes arts and craft activity boxes to 25 teenage and children’s cancer wards across the UK.

Each box contains a range of activities to provide young patients with relaxation, distraction and sense of achievement during long stays in hospital.

If you work for a cancer ward, hospice or treatment centre in the United Kingdom that would benefit from receiving a box, please call the office on 01434 602043 or email
